Regulation 7.2.2: The licensee shall ensure that all new staff and auxiliary staff, prior to working with children in the CBCCPP; and any new individual designated as the representative of the licensee shall submit a Records Check Authorization form to the Division and shall submit to fingerprinting at a location designated by the Division after having received the Fingerprinting Authorization Certificate by the Division.
“Staff A was required to submit to fingerprinting through the Child Development Division in October of 2025, they completed fingerprinting in June of 2026, after this licensing vist was conducted. This is a repeat violation and was cited on 09/03/2025. Corrective Action: Please create a program improvement plan that 1) identifies who will ensure staff are complete fingerprints when the Division sends the Fingerprint Authorization Form 2) How the program will ensure this plan is implemented and new staff are completing fingerprinting 3) how the program will track monitor BFIS to ensure results are received and staff who do not have valid fingerprints are left alone with children.”

Regulation 5.10.1.1.2: Children shall be protected from choking hazards presented by cords on window coverings, electric or electronic appliances or equipment and telephones.
“During the visit, it was noted that each of the prechool classrooms had one cord that was accessible to children that may present a choking or strangulation hazard. In one preschool classroom, there was a blue cord accessible to the children's play space by the teacher's desk that was not secured. In the other classroom, there was a long white cord to a ceiling-hung, moon lamp that was accessible in the children's main playspace and was not secured. Corrective Action: Immediately plan to secure all cords that are accessible to children and may present a choking or strangulation hazard. Develop a system which ensures that all preschool classrooms and areas where preschool children play, eat or sleep are scanned for hazards regularly and all hazards are addressed before children utilize those areas.”

“A review of children's files reflected that the program does not have onsite paper or electronic files that maintain all of the information listed in this rule. Some information is kept electronically in the Power School app, while other information is sometimes found in paper files in classrooms, or was not available at all. While the Power School app has the ability to maintain all required information, it does not appear to be utilized in that manner at this time. Corrective Action: Immediately plan to review all files for preschool children and ensure each child has either an electronic or paper file which contains, at a minimum, all of the information listed in this rule. Develop a system which ensures each child's enrollment file is checked for completion prior to enrollment and that all required information is stored in a paper or electronic file that can be made readily available for review moving forward.”

This staff was witnessed alone with children, when they took a child out of the classroom to use the bathroom and was left alone with the group of children while Staff B took children to the bathroom which is not in the classroom. Corrective Action: Immediately submit a record check authorization form to the Child Development Division and have this staff member complete the fingerprint process to include results before they are left alone with children.”
