Documentation, verification of qualifications, and all annual professional development activities as specified in the rules in sections 7.3 and 7.4 of these regulations shall be submitted to Northern Lights at CCV to be verified and maintained in BFIS.
“A review of staff BFIS credential accounts and staff files reflected that sixteen of the twenty-eight listed staff and auxiliary staff at the time of the visit had not yet submitted required doumentation to Northern Lights at CCV. All sixteen staff had been working with the program for six months to eight and a half years. Corrective Action: All documents were sent to Northern Lights at CCV by June 6, 2025. Develop a system which will ensure that all required documents are sent to Northern Lights at CCV within six month of hire and in a manner which ensures up-to-date BFIS accounts for all staff moving forward. Include details of what system will be utilized, who will be responsible for maintaining the system and ongoing communication with staff, and who will ensure that documents are submitted to Northern Lights at CCV in a timely manner, uploaded to accounts as expected, and/or maintain ongoing communication with Northern Lights at CCV until such a time when submitted documents are reflected in staff credential accounts as expected.”

Regulation 3.3.5: The licensee, staff, and auxiliary staff shall be trained in prevention, identification and mandatory reporting of child abuse and neglect.
“A review of staff BFIS accounts and staff files reflects that five (5) Auxiliary staff, who have worked with the program for four (4) months to four and a half (4 1/2) years, had not yet completed the required Mandated Reporter Training, Protecting Vermont's Children: Reporting Abuse and Neglect. It was confirmed that these staff members do work within the program and have access to children. The director stated in an email that they had removed the listed auxiliary staff as they were not able to provide the document. As the individuals previously worked inthe program, this action does not meet the intent of the rule. Corrective Action: The listed auxiliary staff were removed from the staff list when documentation of the required mandated reporter training, Protecting Vermont's Children: Reporting Abuse and Neglect. Develop a system which ensure all newly hired staff and auxiliary staff complete the required training prior to working with children moving forward. Include details of what system will be used, who will be responsible for maintaining the system and ongoing communication with staff, and who will be reqponsible for ensuring professional development documents are sent to Northern Lights at CCV in a timely manner moving forward.”

“The associated parties list was reviewed with the program and it was found that there were eleven (11) individuals on the list that have not been present since June 2022. This is a violation because the associated parties list still lists the individuals that have not been present and the list was not updated within five (5) working days. Corrective Action: It is required to review the associated parties list and remove any individuals that are no longer present. It is required to develop a system that ensures the associated parties list is kept up to date in the future.
