“The center submitted a personnel list in January 2024 that included 2 staff persons who began employment in November 2023. The center must submit new staff notifications to the licensing specialist in a timely manner. At the time of the inspection, two persons on the AP list were no longer employed since June 2023. The center must also ensure that the licensing specialist is notified when a person ends employment at the center. The staff change form was completed at the time of the inspection..”

Please have the parents obtain the documentation of lead blood tests from the physician and add to the children's files.”
