“A staff member who has worked in the program since April 2023 did not complete the fingerprint process for the Child Development Division. The fingerprint letter from CDD was issued on 4/3/2023 requesting this staff member to complete the fingerprint process. The new directors took over around the same time and assumed the staff members' fingerprints cleared. Corrective Action: Immediately have this staff member fill out a new record check authorization form and submit the form to the Child Development Division. By 7/20/2026, have this staff member complete the fingerprint process and submit the fingerprint reciept to this Licensor.”

Regulation 5.1.2: General Health Examinations: Within forty-five (45) days of enrollment, the licensee shall obtain documentation of the child's age appropriate well care exam from the parent. Documentation shall include information regarding any health conditions and medications that may impact the care of the child.

“During a review of children's files, it was noted that 75% of the files reviewed, did not contain General Health Exam Forms. The Director confirmed that all of the children whose files were reviewed have been enrolled for at least forty-five (45) days. The Director stated that while she has requested these forms from families on multiple occasions, after her recent review of children's files, approximately fifty (50) percent of the enrolled children did not yet have General Health Exam forms on site. Corrective Action: Immediately plan to collect General Health Exam forms for all children who have been enrolled for at least fortyfive (45) days. Develop a system which will ensure General Health Exam forms are collected for each newly enrolled child in a timely manner moving forward.”

Regulation 7.4.4: All staff shall complete fifteen (15) clock hours of annual professional development activities as required in the rule 7.4.2 of these regulations. Substitutes filling a staff position for less than thirty (30) consecutive days within a 365 day period shall be exempt.

“A review of staff BFIS Quality and Credential Accounts and Staff Files reflected that Staff A, B, C, and D, had not completed 15 hours of annual professional development for the last full year based on each staff member's start date. Staff A confirmed that staff A, B, C, and D had not completed 15 hours of annual professional development based on individual start dates due to the stress that the COVID-19 pandemic placed on staffing. Corrective Action: Effective immediately, plan to have Staff A, B, C and D complete 30 hours of annual professional development for their current year based on individual start dates. Develop a system which will track staff start dates and completion of annual professional development in a manner which will ensure that all staff complete a full 15 hours of professional development each year moving forward.”
