“Upon enrollment of a child, RECDC programs shall require that the person enrolling a child in the program present proof of the child's identity and age. The program did not have documentation that the person enrolling Child #1 presented proof of the child's identity and age upon enrollment. Child #1 has been enrolled for over a year. The program did not have documentation that the person enrolling Child #3 presented proof of the child's identity and age upon enrollment. Child #3 has been enrolled for over nine months. The program did not have documentation that the person enrolling Child #5 presented proof of the child's identity and age upon enrollment. Child #5 has been enrolled for over a year.”

Child #1 did not have documentation of immunizations on file. Child #1 has been enrolled for over a year. Child #2 did not have documentation of immunizations on file. Child #2 has been enrolled for five months. Child #3 did not have documentation of immunizations on file. Child #3 has been enrolled for nine months.”

“The required ration of staff to children for ages 36 months to five years is one staff member to 10 children. During the inspection, there were 12 children present with one staff member in Room 7/8 (3 to 4 year olds).”

“All volunteers shall provide a sworn statement, submit to fingerprinting and authorize the center to obtain a copy of the results of a search of the central registry. Three volunteers were observed in classrooms during the inspection. Staff #1 confirmed that there were no background checks on file for the volunteers.”

Caregiver #1?s(start date: 02/19/2018) most recent fingerprinting results was dated 02/13/2018. 2. At the time of inspection, Caregiver #2 (start date: 09/06/2017) did not have a copy of fingerprinting results on file. 3. 2. At the time of inspection, Caregiver #3 (start date: 01/02/2019) did not have a copy of fingerprinting results on file.”
