“Upon enrollment of a child in a regulated child day program, such child day program shall require information from the person enrolling the child regarding previous child day care and schools attended by the child. There was no documentation of previous child day care or schools attended in the six children's records that were reviewed.”

“Based on observation and staff interview the center did not establish and implement procedures for hand washing by children before eating. Evidence: Staff was observed putting hand sanitizer in children’s hands, before lunch, rather than having them wash their hands. The director confirmed they use hand sanitizer because it is quicker.”

“Based on record review and staff interview the center did not ensure to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. Evidence: Staff #1 and Staff #3 indicated on the sworn statement as to having lived outside of the state of Virginia within the last five years. The staff person in charge confirmed that the center had not obtained all required out of state background checks. 1. There was no documentation of an out of state (Louisiana) criminal record check and an out of state child abuse and neglect search in the record for Staff #1. 2. There was no documentation that an out of state (Florida) child abuse and neglect search in the record for Staff #3.”
