“The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. On September 15, 2026, the center did not follow its Injury Prevention Plan regarding playground supervision. The plan requires all teachers to be spaced out across the playground, continuously circulating, interacting with children, scanning the play area, and maintaining a resilient surface to ensure safety. However, during the initial observation, two of the three staff members were seated in chairs while children were actively playing on playground equipment. Additionally, the resilient surface was not maintained as required, creating a potential safety hazard.”
