“Children under 10 years of age always are required to be within actual sight and sound supervision of staff at all times. On 4/27/2026, at approximately 4:45pm, one of the Preschool classes was transitioning out of the elevator and over to the playground. Staff A had 10 children in the elevator and conducted a name to face count as each child exited the elevator to the 6th floor where the playground is located. Staff A observed all 10 children exit the elevator and then went to open the next door leading outside to the playground area. As Staff A was opening the next door, which is a few feet from the elevator, 4 children (Child A, Child B, Child C, and Child D) ran back onto the elevator. Staff A tried to get to the elevator, but it closed before Staff A could get there and the elevator went down to the second floor (which is the lobby of the childcare center). When the elevator opened in the lobby, there were parents standing there who were waiting for the elevator to come down. The parents then notified Staff B, who was in the lobby to sign out for the day, that there were children on the elevator. Staff B went and got the 4 children, 3 out of 4 of them were crying. Staff C was heading down the stairs to see what happened. Staff C talked with the children to help console them and then took the 4 children back up to the playground to return to their class.”

“When staff are supervising children, they are required to always ensure their care, protection, and guidance. When Staff A was transitioning 10 children from the Preschool classroom on the 3rd level of the building to the playground on the 6th level of the building, 4 of the 10 children ran back onto the elevator and the doors closed. The elevator then went down to the second floor, which is the lobby of the childcare facility. Staff B then received the children and Staff C helped to return the children back to their classroom. For approximately 20 seconds, Child A, Child B, Child C, and Child D were not under direct supervision of any staff member and staff could not ensure their care and protection during this timeframe.”

“Children under 10 years of age are required to always be within actual sight and sound supervision of staff. On 10/27/2025, shortly after 11:30am, Staff A was outside on the playground located on the 4th floor/level of the building. Staff A began to transition the 10 children in Staff A's care into the elevator to go back to the 2nd floor where their classroom was located. Staff A got all 10 children in the elevator and arrived on floor 2. When the doors opened, multiple children began to run down the hallway towards the classroom. Staff A tried to keep a foot in the elevator to keep the door from closing, but Staff A's foot slipped and the door to the elevator closed with 3 children still inside. Staff A pushed the button twice for the doors to open, but they did not open. Staff A stated that they did not have a way to contact management at that moment and began following the children going down the hallway towards their classroom. Approximately 30 seconds later, Staff B, who was sitting at the front desk area in the lobby heard children banging on the wall and calling out from the elevator on the 1st floor. Staff B opened the elevator door and found the 3 children (Child A, Child B and Child C) that were left behind. Two of the children were crying as Staff B reassured the children that they were safe. Staff B then returned the 3 children to the classroom where Staff A was.”
