    • Health, medication and illness

Regulation 13.18: Written parental permission shall be obtained before administering any medication. The permission statement shall include the name of the child, name of the medication, dosage, the date and time to be given and how the medication is to be given (mouth, ear, etc.).

“The program did not have a parent permission form or an allergy plan signed by the doctor and parent. The child has an Epi Pen for a bee allergy and was in care during this compliance visit. It was confirmed by the parent that they had not filled out a parent permission form releasing the staff to administer the medication for the child during this licensing visit. Despite the Epi Pen not being administered, it is an emergency medication and would need to be administered at a response that could not be delayed while seeking parental permission which would have been verbal at best. Corrective Action: Immediately have the parents sign a medication permission form and have the allergy plan for staff to administer the medication if needed.”

    • Staff training and qualifications

Regulation 5.35: The Program Administrator or designee shall ensure a minimum of 8 hours of annual Professional Development Activities for new and returning Afterschool Child Care Program Staff related to the attainable goals defined in each Staff's Individual Professional Development Plan (IPDP). This training is in addition to any infant/child CPR certificate or recertification and First Aid training.

“A review of the staff's BFIS Quality and Credential account, staff file, confirmation from a review of Northern Lights and confirmation from the director, that one program member staff completed three (3) hours of the required eight (8) hours of professional development between 11/10/24 to 11/10/2025. Corrective Action: By 5/22/2026, have this program staff complete the missing five (5) hours of professional development. These five (5) hours will not count toward the professional development needed by 11/10/2026.”

    • Supervision and ratios

Regulation 6.3: The staff/child ratio shall not exceed 1:13. When the number of children exceeds 10, another identified Staff person shall be available within immediate proximity on site in order to provide prompt assistance in the event of an emergency.

“The attendance, staff schedule and the head count of children and staff showed the program operated out of compliance with staff/child ratio from 2:20 to 2:45 during this licensing visit on 5/6/2026. The program signed in forty-one (41) students with three (3) program staff. One (1) child signed out at 2:30 and the second (2) child signed out at 2:45 bringing the program back into compliance with staff/child ratio at 2:45. Operating out of compliance with staff/child ratio is considered a serious violation. Corrective Action: Immediately operate in compliance with staff/child ratio. By 5/22/2026, submit a program improvement plan with steps the program will take to maintain staff/child ratios. A Parental Notification Letter (PNL), which is a copy of the licensing report, must be provided to each parent of every child enrolled within ten (10) days of the report submission. Provide a list of parent's names, child's name and contact information for each enrolled child to this Licensor.”
