Regulation 3.7.2.2: The licensee shall ensure that evacuation drills are conducted at least once a month, and children and staff are evacuated in under three (3) minutes. Licensees of a CBCCPP located within a public or independent school building may count a lock-down drill performed while the CBCCPP is in operation in place of a monthly evacuation drill with the CBCCPP children and staff and at least three (3) of the monthly drills conducted within 365 days shall be evacuation drills.
“The evacuation drill log noted that a drill was completed in October 2025. No drills were documented for September, November, and December 2025. Discussion with staff confirmed that drills were not done in those months. Corrective Action: Complete one (1) evacuation drill per week until 2/9/2026. Regulation 3.3.5: The licensee, staff, and auxiliary staff shall be trained in prevention, identification and mandatory reporting of child abuse and neglect.
“During an annual compliance visit conducted on 03/20/2023, a violation was cited as eight (8) individuals listed on the program's BFIS staff list had not yet completed the required mandated reporter training and two (2) of those staff were witnessed to be left alone with children during that visit. Corrective action assigned for that violation required that all eight (8) staff complete the required mandated reporter training in a timely manner. As part of that corrective action, the program was also required to submit a plan for coming into compliance and remaining in compliance with this rule moving forward. As of 06/05/2023, the Division had not yet received this corrective action. While the director was able to provide documentation reflecting that seven (7) of those staff had completed the required training during this follow-up compliance visit on 06/05/2023, the Director was not able to provide documentation that one (1) of the eight (8) staff members had completed the madated reporter training as required by this rule and the assigned corrective action. On 06/05/2023, the Director submitted an incomplete corrective action plan for the original violations cited on 03/20/2023. This plan did not include required information about a system for tracking the completion of required trainings by newly hired staff, ensuring staff are not left alone with children until this training is completed, and remaining in compliance moving forward. This is a repeat violation. Corrective Action: On 06/06/2023, the Director submitted the mandated reporter training certificate of completion with a completion date of 06/06/2023 with a cover sheet to Northern Lights at CCV. This satisfies the requirement for all current staff to complete the required mandated reporter training. As the corrective action plan submitted on 06/05/2023 did not satisfy the corrective action assigned for the original violation cited for the site visit on 03/20/2023, develop a system which ensures that all newly hired staff take the required mandateed reporter training in a timely manner and prior to working with children and submit the documentation of completion to Northern Lights at CCV in a timely manner moving forward. Include in the system, details of: 1. what ystem will be utilized to track the progress of completing required trainings for newly hired staff, 2.who will be responsible for maintaining the system, 3. who will be repsonsible for making staff aware of and ensuring staff follow-through with the requirement to complete the training prior to working alone with children and submit the documentation of completion to Northern Lights at CCV in a timely manner, 4. the timeframe staff will be given to complete the training, 5. who will provide oversight and ensure that no staff work with children until they have completed the required training moving forward.”

Regulation 7.2.4: The licensee shall ensure that no person shall be left alone with children without approval from the Division which shall be based on the results of the background check to include fingerprinting.
“During the visit, Staff A and Staff B were left to supervise 8 children. A review of staff BFIS accounts and staff files reflected that neither Staff A or Staff B had received approval to be left alone with children from the Division based on the results of the fingerprint supported background check. A further review of staff BFIS accounts and staff files reflected that 5 additional staff who had received Finger Print Acquisition letters from the Division between 4 and 11 months prior to the visit, had not yet completed the fingerprinting and/or out of state records check process or received a final clearance letter from the Division. The Director confirmed that these 5 individuals are currently active staff within the program. Corrective Action: Immediately plan to ensure that no staff be left alone with children without having recevied approval from the Division based on the results of a fingerprint supported background check. Immediately ensure that all current staff complete all necessary steps of the fingerprint supported background check, including out of state records check as applicable. Develop a system which will prompt you and/or staff members to submit to fingerprinting and/or out of state records checks, as applicable, in a timely manner upon hire and at least once every 5 years thereafter. Include in your system: 1. details of the system that will be utilized, 2. who will be responsible for developing and maintaining the system, 3. how staff will be notified of their responsibility to take the proper steps, 4. who will follow-up with staff and ensure follow-through with the process in a timely manner, 4. who will ensure clear and consistent communication with staff pertaining to when they can and cannot be left alone with children moving forward.”

The 8 staff have been employed with the program between 4 and 20 months. Corrective Action: Immediately plan to have the 8 staff complete the required mandated reporter training; Protecting Vermont's Children: Reporting Abuse and Neglect, and submit their documentation of completion with cover sheets to Northern Lights at CCV(NLCCV). The required training can be found on the Divsion's website: dcf.vermont.gov. Follow the path: Divisions>Child Development Division>Providers>Professional Development. Develop a system which ensures that all newly hired staff take the required mandated reporter training in a timely manner and prior to working with children and submit the documentation of completion to NLCCV in a timely manner moving forward. Include in the system the details of: 1. what system will be utilized, 2. who will be responsible for maintaining the system, 3. who will be responsible for making staff aware of the requirement and ensuring staff follow through with the requirement, including NLCCV submission, in a timely manner, 3. the timeframe staff will be given to complete the training, 4. who will ensure that no staff work with children until the required training is completed moving forward.”
