“Complaint alleged the center was out of ratio on Wed AM, 9-23-26. Complainant stated the aide who is normally at the center was sent to another center that morning and there were two staff members present with more than 30 children. LS arrived at the center on 9-25-26 and reviewed the AM attendance record for 9-23-26. There were two staff present that day, the Director and the lead school age teacher. At 8:59am, the 31st and 32nd children arrived to the center, putting the center out of compliance for staff child ratio and group size. There were 32 children present with two staff members from 8:59 am to approximately 9:15am when the children were dismissed to their classrooms. The center is reminded that staff child ratio for school age children is 1 adult to every 15 children and group size, before separated, should not exceed 30. There are normally 3 staff members at this site in the AM hours. Please send a written plan of correction to LS. When LS was present at the center today, there were 3 staff members present with 25 children and the center was in compliance for staff child ratio and group size.”

“The Director stated the school's principal has made the decision to move the child care children to the Media Center for approximately 20 minutes in the afternoon while the school's 'car riders' are dismissed from the cafeteria. The Director stated this has been in place for the last 3 weeks or so and that this will be permanent for the school year. The Media Center has not been approved for use by the Office of Child Care and should not have been used without prior approval of the Office of Child Care. If this space is to be added to the license, please send a change in child care facilities form to the Licensing Specialist requesting this room be approved for child care use. The Licensing Specialist did take measurements of the Media Center while on site today and has no objection to adding it to the license once the change has been requested by the CCPS Child Care Specialist. Please remind school personnel that the child care children are only supposed to be in rooms that are currently approved and noted on the child care center's MSDE Office of Child Care license..”

The child has the Kaiser Permanente equivalent of the MSDE health inventory Part 2 form, but Part 1 of the MSDE health inventory form or equivalent was not on file during the inspection today. Please obtain from the parent and add to the child's file. Send a copy to the Licensing Specialist.”

“The third staff member at this site was moved to a different site at the beginning of the school year and there has been a substitute in place. Today, the substitute was a child care teacher from the Plum Point Elementary BASP site. The substitute at this site was not recorded on the substitute record. When substitutes are used, even if they are qualified child care teachers, and they are not on the staffing pattern at the site or a regularly scheduled staff member, they must be recorded on the substitute record. This was corrected while the Licensing Specialist was on site.”

“There is one box of Zyrtec present in the medication box for a child with an allergy. There is a current medication form signed by the health care provider and parent for the Epi Pen that is also present in the box, but the medication authorization form for the Zyrtec was for last school year and is not current. If the child needs to keep the Zyrtec at the child care program, please have the parent obtain an updated medication administration form for this medication, which is signed by the health care provider and the parent. If this medication is no longer needed at the child care program, please return it to the parent. Please send a written plan of correction to the Licensing Specialist. There are two boxes of Albuterol for two other children that have current medication administration forms. There is no expiration date on the Albuterol, but the refill date is 7-2023. Please check with the parent to insure these medications have not expired.”
