The provider stated that she had not submitted any educational documentation to Northern Lights. Corrective Action: By 9/7/2021, submit educational documentation to Northern Lights at CCV with the required cover sheet. The Verification Cover Sheet can be found at: https://northernlightsccv.org/resource/verification-cover-sheet/ .Submit confirmation to ann.sparrow@vermont.gov that all required information have been sent to Northern Lights at CCV.”
