AAC R9-3-505.D.2. D. A certificate holder shall ensure that: 2. A glass window, mirror, or other glass surface that is located lower than 36 inches above the floor, a sliding glass door, or another type of glass partition that is located lower than 36 inches above the floor: a. Is made of safety glass that has been manufactured, fabricated, or treated to prevent the glass from shattering or flying when struck or broken; b. Is shielded by a barrier to prevent impact by or physical injury to an enrolled child; or c. Has conspicuous markings located at a child’s eye level;

“Findings: Based on Compliance Officer #1's observation, it was determined that there were no markings on the sliding door that opened to the backyard.”

AAC R9-3-401.B.2. B. A certificate holder shall ensure that: 2. The areas of the child care group home approved for providing child care services are maintained free from hazards;

“Findings: Based on Compliance Officer #1's observation, it was determined that in the backyard, there was flaking cement along the west wall that was accessible to the enrolled children.”

AAC R9-3-504.C.2. C. A certificate holder shall: 2. Ensure that each rechargeable fire extinguisher in the child care group home’s residence: a. Is serviced at least once every 12 months, and b. Has a tag attached to the fire extinguisher that specifies the date of the last servicing and the identification of the person who serviced the fire extinguisher.

“Findings: Based on Compliance Officer #1's observation and review of facility documentation, it was determined that the 1A-10-BC fire extinguisher was not serviced in the past 12 months.”
