“A preschool aged child became separated from their group during the transition from the classroom to the playground. Adults became aware after the group had entered the playground when a staff member observed the child walking toward the playground with two adults who were not staff. The path from the building to the playground is down a sidewalk that abuts a service road between two parking lots. The child was seen by staff before they crossed the road to the playground. Corrective Action: Develop a plan to ensure that children are accounted for during transitions and train staff on new practices to minimize the risk that this could happen again.”
