“Findings: Based on facility documentation and the Compliance Officer's observations, it was determined that the Criminal History Affidavit for Staff #1 (start date 10/01/2022) and Staff #2 (start date 08/01/2022) lacked the second page of the form. It was determined that the files for Staff #1 (start date 10/01/2022) and Staff #2 (start date 08/01/2022) lacked a DCS Central Registry Clearance Form (Staff #1 dated 11/15/2022, Staff #2 dated 11/14/2022) that was completed prior to the staff's start date as required.”
