“The religiously exempt program is required to disclose to parents in writing the fact that it is religiously exempt from licensure and post the fact that it is exempt from licensure in a visible location on the premises. Evidence: The record for Child #1, Child #2, Child #3, Child #4 and Child #5 did not contain documentation of a written disclosure signed by the parents that the program is religiously exempt and there was no posting that the facility is exempt from licensure.”

“Based on staff interviews and staff record reviews, the facility did not ensure that staff were trained to perform daily simple health screenings and exclusion of sick children. Evidence: reviewed 4 staff records and spoke with the director and found that none of the staff were trained in daily health observation.”
