The sink located in the girls restroom supplied warm running water that exceeded 120 degrees Fahrenheit. It was measured 122 degrees Fahrenheit.”

“Based on staff record review, the center failed to obtain results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening. Evidence: 1. The record of staff #2 did not contain an updated tuberculosis screening or test. The date of the tuberculosis screening in the file was dated 11/15/2021.”

“Based on interview and review of the injury prevention plan, the center failed to update the injury prevention plan at least annually. Evidence: 1. The Injury Prevention on file was last dated August 28, 2021. 2. Staff #1 verified that he injury prevention had not been updated in 2022.”

“Based on observation, the center failed to maintain all areas in a safe and operable condition. Evidence: 1. In the Green Classroom there was a white and blue shelve in that was not stable or sturdy. The shelve was leaning to the right and would not stand straight. 2. In the upstairs girls restroom, in the bathroom stalls there were two large holes in both of the stalls right below the toilet paper holder. The stall on the right had a large patch of peeling paint. 3. Upstairs in the foyer area between the restrooms and the Purple Classroom there were several areas on the wall that had peeling paint.”

“Based on review of children records and medications, the center failed to obtain a written care plan for a child with a diagnosed food allergy. Evidence: 1. Child #1 had an emergency medication, (Epi-Pen), at the center due to a diagnosed food allergy. There was not documentation of a written care plan on file for Child #1. 2. Administration verified that the written care plan for Child #1 could not be located.”

“Based on observation, hazardous materials were not kept in a locked location. Evidence: 1. In Classroom #1 there was a bottle of Juniper Surface Cleaner with Clorox Bleach setting on a shelf in the closet. The closet was not locked. 2. In Classroom #2 there were two containers of Clorox Disinfectant Wipes sitting on the table as you enter the classroom. The table was accessible to children. 3. Both the Juniper Surface Cleaner, and Clorox Disinfect Wipes stated to "keep out of reach of children". 4. Staff verified that the cleaning materials were not kept in a locked location.”

An injury report dated 11/12/2021 was missing the method of notification, and prevention for reoccurrence. 2. The injury reports dated 12/03/2021 and 12/10/2021 were missing the date and time the parent was notified. 3. Staff verified that the reports did not contain all the required information.”
