“Based on interview and review of documentation, the center failed to ensure that the medication authorization is available to staff during the entire time it is effective. Evidence: 1. Child #6 had an epinephrine pen and an allergy action plan on site. The allergy action form indication that the epinephrine pen was effective at the time of the inspection. There was no medication authorization. 2. Staff #1 confirmed there was no medication authorization.”

“Based on observation and interview, the center failed to ensure that children’s hands were washed with soap and running water or disposable wipes after eating a snack. 1. 6 children were observed to not wash hands after eating snacks. 2. Staff #1 and #2 confirmed that the children did not wash their hands after eating snacks.”

“Based on observation and interview, the center failed to ensure that resilient surfacing complied with the minimum safety standards of 6 inches for wood chip mulching. Evidence: 1. Less than 2 inches of wood chip mulch was observed in the fall zones around the climbing structures on the playground. 2. Staff #1 confirmed there was not 6 inches of wood chip mulch around the climbing structures.”

“Based on measurements taken, there was not the required six inches of resilient surfacing in two areas around one piece of play equipment. Evidence: 1. In the fall zone of the large climbing structure the mulch measured to be approximately 3 to 4 inches in front of the slide and 2 to 3 inches in front of the ramp.”
