      “There are three (3) staff who are present at the program and are not on the program's BFIS Associated Parties list but are on the Associated Parties list for other Essex Recreation and Parks programs and have full background clearances. There are seven (7) staff who are still on the program's BFIS Associated Parties list who no longer work at the program with end dates greater than five (5) working days from 12/17/2021. Corrective Action: Immediately submit record check authorization forms for the three (3) staff who are not reflected on the program's BFIS Associated Parties list. Create a system to ensure that the program's BFIS Associated Parties list is kept up to date when onboarding and offboarding staff.”
