“Based on a review of the staff handbook and interview, the center failed to establish and implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards. Evidence: 1. In the Purple Dino Room (ages 2 1/2 to 3 years) the paint is peeling on the baseboard trim. 2. In the Green Turtle Room (ages 3 to 4 years) Off insect repellant was in a basket on a low shelf accessible to children.”

Evidence: 1. The record for staff 3 was reviewed. The hire date is documented as 1/2/23. The staff health report on record is dated 3/31/23. 2. The director verified the date of hire and staff health report.”
