    • Building, equipment and play areas

Regulation 5.10.3.1.5: Fencing installed after the effective date of these regulations shall: * Be a minimum of four (4) feet in height with openings no larger than 3.5 inches; * Equipped with gates with self-closing and self-latching mechanisms; and * Include at least two (2) exits with one (1) that does not lead into the CBCCPP.

“The Preschool classroom utilizes two (2) playgrounds. A small playground at the front of the school and a larger playground at the back of the school. There are two (2) gates on each playground. One (1) gate on the large playground has a five and a half (5.5) inch gap between the two (2) sections of gate and a six (6) inch gap between the bottom of the gate sections and the ground. On the small playground, one (1) gate has a gap of five (5) inches between the bottom of the gate and the ground, and the second gate has a gap of five and a half (5.5) inches between the bottom of the gate and the ground. Corrective Action: The large playground shall not be used until the gaps in the gate are fixed. During the 09/04/2026 the program was asked to push the pea stone under the gates to decrease the gap. The program must monitor the depth of the pea stone to ensure the gap does not change. Please send photos of the gates once the gaps are closed. Status: Appeal Period.”
    • Supervision and ratios

Regulation 6.2.6.1: Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.

“On 09/02/2026 Child X was observed by a member of the public walking on the sidewalk away from the school. At the time of the incident there were eleven(11) children and three(3) staff members present on the playground. Staff believe that Child X slipped through a gap in the playground gate. Staff believe this is the case because Child X attempted to slip through the gate after they were back in the playground. The program promptly notified the Child Development Division and Child X's parent. Staff increased their supervision of Child X. Child X just turned three(3) and this was their second day in attendance. The Preschool classroom is a mix of newly enrolled children and children who attended last school year. This is a serious violation. Corrective Action: For the first thirty(30) days of the school year, one staff member will be assigned to supervise the newly enrolled children when they are on the playground. During this thirty (30) day period staff will observe and note if the newly enrolled children need additional support while on the playground. .A Parental Notification Letter (PNL), which is a copy of the licensing report, must be provided to each parent of every child enrolled within ten (10) days of the report submission. Please provide this licensor with the parent contact information for all children enrolled at the program. Status: Appeal Period.”

    • Staff training and qualifications

Regulation 3.3.5: The licensee, staff, and auxiliary staff shall be trained in prevention, identification and mandatory reporting of child abuse and neglect.

“A review of staff BFIS Quality and Credential accounts show that three (3) staff who have been employed at the program for over a month have not completed the required Protecting Vermont's Children Mandated Reporter training. Brenda confirmed that they have not yet taken this training. Corrective Action: All individuals must complete the required training by 4/2/2021. Submit verification of completion to Northern Lights at CCV and the date training was completed to this licensor.”

“Brenda confirmed that three (3) staff members who have been working at this program for more than 3 months are not currently certified in first aid and CPR. Corrective Action: By 4/2/2021 ensure that staff enroll in or complete CPR and first aid training.
