The green play structure located outside on the playground had multiple areas that are rusted and have sharp edges. 2. The wooden structure above the gardening area on the wall is broken in multiple areas and has a protruding nail sticking out. 3. Multiple silver spikes that are located on the black framing that surrounds the play structures were observed to be protruding from the frame.”

From approximately 11:42am-11:44am, Staff A was observed to be alone with 22 children. Staff A stated that the assistant teacher was helping one child in the restroom area, which is located outside of the classroom and down the hallway.”
