The results of the central registry search were received six months after date of employment. Staff #3, employed for over nine months, did not have documentation of a central registry search request prior to employment and did not have results of a central registry search.”

“Systemic Deficiency: Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Staff #1, employed for over nine months, had a staff health form that expired four months ago. Staff #2, employed for over nine months, had a staff health form that expired four months ago. Staff #3, employed for over nine months, had a staff health form that expired three months ago.”

“The center shall establish and implement procedures ensuring that a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. Staff discussed procedures concerning daily health screenings. There were no staff at the center that were formally trained in daily health screenings at the time of inspection.”
