“LS observed missing MA form for a child recently diagnosed with diabetes. The medication is only as needed and the provider has not had to administer it as of yet but already has the medication since it travels with the child. The provider called the parent to inquire about the form and the parent stated the MA form was completed but she submitted it to the school. LS explained that the MA form submitted to the school can also be provided to the provider for if the child needs to come for BAS care. Provider is to send a picture of the MA form as proof of correction to OCC ASAP.”
