“A Preschool Teacher should have completed 12 hours of continued training by 09/2026. She only completed 11 hours of training. Please have her completed the additional one hour of continued training that is required as soon as possible. After completion, send a copy of the certificate of completion to OCC for the correction of this non-compliance.”

“During a review of a sampling of children's records, no evidence was found that a 3 year old child has the results of any blood lead test in his file. The BLT sheet says "pending". Please obtain the blood lead test results for the identified child and submit them to OCC for the correction of this non-compliance.”

“During a sampling of children's records, it was found that one 2 year old child's parent did not date the emergency form. Another 2 year old's parent did not sign or date the emergency form or list the child's physician's address on the emergency form. A 4 year old child's emergency form did not have the physician's address. Two 3 year old children's emergency form did not include any physician information at all. Please inform the parent's of the missing information so they may add it to their respective child's emergency form and initial and date the form to indicate the updated information. Send a copy of the updated emergency forms to OCC for the correction of this non-compliance.”

“During a review of staff files, it was found that volunteers SF and JK have not had a medical completed. Please have each of these staff members have a medical completed on the OCC medical form and send it to OCC for the correction of this non-compliance.”

“During a review of all of the children's emergency forms, it was found that 24 out of the 72 emergency forms are missing information to include the child's correct date of birth (JS & GB), people who are authorized to pick up the child, physician information to include name, address and phone number, emergency contact phone numbers and one is missing her parent's phone numbers (SC). Three emergency forms are missing a yearly update. The Center was given a copy of the Health Record Review form so the Director will know which child needs what information. Please have the respective parents add the missing information to the emergency forms, initial and date it for the update. Please send the identified emergency forms with the added information and update to OCC for the correction of this non-compliance.”
