    • Staff training and qualifications

Regulation 3.4.7.3: Within six (6) months of the initial date of employment; staff and auxiliary staff left alone with children and/or counted in staff/child ratio as specified in the rule 6.2.1.8 of these regulations, and the program director shall maintain an up-to-date BFIS Quality and Credential Account. Documentation, verification of qualifications, and all annual professional development activities as specified in the rules in sections 7.3 and 7.4 of these regulations shall be submitted to Northern Lights at CCV to be verified and maintained in BFIS.

“A review of staff BFIS credential accounts and staff files, reflected that many staff documents were not yet reflected in accounts. While the director stated the program's process for submitting documents with cover sheets to Northern Lights at CCV, it was noted that the cover sheet utilized was not the most recent version. Further, the program does not appear to have an established system for conducting regular checks of staff BFIS accounts to ensure all submitted documents have been added or following up with the advisors at Northern Lights at CCV when items do not appear in accounts as expected. This is a repeat violation. Corrective Action: Immediately plan to: 1. review all staff BFIS quality and credential accounts, 2. record all outstanding required documents, including annual professional development, approved orientation training and mandated reporter training certificates, current CPR and first aid certifications, and staff qualification documents for their titles as listed in section 7.3, 3. resubmit all documents that are not yet reflected in staff BFIS accounts with the most recent version of the NLCCV verification cover sheet, 4. set a reminder and check staff BFIS accounts in approxiately thirty days from submission, 5. maintain communication with Northern Lights at CCV until such a time when all issues have been resolved and all documents are reflected in accounts as required. The most current version of the verification cover sheet can be found on the Nortrhern Lights at CCV webpage northernlightsccv.org. Your Northern Lights at CCV resource advisor is Adri Taylo-Behrman. Adri can be reached at adri.taylo-behrman@ccv.edu or 802-654-0730.”

Regulation 6.2.6: Supervision of Children Staff shall ensure visual supervision of children, except as allowed in rules 6.2.6.1-6.2.6.4 of these regulations.

“On 02/11/17, when 15 preschoolers were being brought inside for lunch time, one child was left alone outside for 5 to 10 minutes before staff realized a child was missing from the classroom. Due to the significant amount of time the child was outside alone the level of risk is high. This is a serious violation requiring a parental notification letter.
