“On January 17, 2025, the provider was observed caring for 3 children under the age of two (1 resident, 1 enrolled child and 1 sibling of an enrolled child). The licensing specialist is on site to conduct a follow-up inspection. At the time of the licensing specialist's arrival, there were 4 child care children present including 2 under the age of two (21 months and 4 months). The resident child under the age of two was present with his parent and went upstairs. One of the grandchildren who reside in the home remained in the child care space (age 3). The provider's assigned capacity is 4 with 1 child under the age of two. The provider has 2 children enrolled under the age of two and both were present today. Attendance sheets for the month of January were obtained and show multiple days when both child care children were present who are under the age of two. The provider stated she thought that the 4 month old could attend as long as the resident 22 month old was not present in the child care space and being cared for by his parent. It was explained to the provider at the January 17th inspection that the resident 22 month old always counts towards capacity.”

“The provider could not produce attendance for any months prior to January. She stated she discarded them as the months had passed. The provider was reminded that attendance records must be kept for at least 2 years from the creation date.”

“The provider has 1 child enrolled that is under the age of two and was present today. There is one resident child who is under the age of two. The provider also had an enrolled sibling's child in care for a couple hours while the mother went to an appointment. The mother is planning on being back by 12 PM. This child is scheduled to start in March after the resident child turns two. The resident child was taken upstairs to be cared for his parent so that the provider is only currently caring for 2 children under the age of two. The provider must not allow the infant sibling to be in care unless space allows; the enrolled child under the age of two must not be present for him to attend. The resident child under the age of two always counts towards the capacity and is the reason the provider's under two capacity is only 1 child. The provider will ensure that she maintains her capacity of children under the age of two at all times.”
13A.15.03.04A(1)
“There are 2 children who do not have an emergency card on file. The provider will ensure the families complete an emergency card and will send a copy to the licensing specialist.”
13A.15.03.03E
“Two of the children need the consumer pamphlet in their file or their parents need to be advised on how to obtain it (such as in the contract).”
13A.15.03.02A
“Immunizations for a three-year-old child were not on file. The enrolled child under the age of two needs documentation of the lead blood test done at 12 months. The two-year-old needs the lead blood test done at 24 months. Please have the parents obtain the documentation from the physician and add to the children's files.”
13A.15.03.05C(1)
“The provider has another grandchild residing in the home who was born in March 2023. The provider did obtain a medical report for the grandchild and a copy was provided to the licensing specialist. The provider's capacity will be reduced to 4 total children because there are now 4 residents under the age of six.”
