      3270.19(a) Required to report

“Based upon interviews with Staff it was determined that on June 12, 2026, Staff Person #1 used physical discipline with Child #1, a preschool age child. It was reported that Staff Person #1 appeared to be frustrated with Child #1 when Staff Person #1 "yanked the child up by the hands from the floor". It was reported that the child did not have their balance and as a result fell back to the ground. Staff Person #1 then "yanked" the child up again by the hands and then aggressively pushed the child into a wooden "cozy cube" that is used for quiet time in the classroom. This wooden cube was observed to have a cut out entrance, and it was reported that entrance was what Staff Person #1 pushed Child #1 through. Other staff present in the classroom did intervene at that moment. The facility failed to report the incident to Childline as mandated by the CPSL.”
