“Hazardous substances must be in a locked place that prevents access by children. In the unlocked cabinet under the kitchen sink, unlocked teacher supply room, and unlocked cleaning supply room there were approximately thirty cleaning and sanitizing products that were labeled "keep out of the reach of children". All of these chemicals were adjacent to the rooms being utilized by the children in care.”

During the inspection, staff #1 and staff #2 were each in and out of the rooms on at least three occasions for several minutes at a time, leaving children out of sight and sound supervision of the remaining staff person.”

At the time of the inspector's arrival at 12:25 pm until approximately 12:35 pm there were 10 children, the youngest being 10 weeks old, in care with staff #2.”

“Cribs, cots, mats and beds used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Out of the seven preschool aged children napping six did not have a bottom cover and one did not have a top and bottom cover.”
