“The center is required to inform the department’s representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. On February 19, 2025 Child E tripped in the classroom and began complaining that their foot hurt. Staff applied ice but the child continued to complain about their foot hurting. After that day, the child did not return to the center for several days, so staff reached out to the family and learned that the child had broken their foot when they tripped. The child returned to care with a boot on their foot shortly after, but the center did not report the injury to the department.”
