    • Supervision and ratios

Regulation 6.2.6.1: Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.

“On 6/13/2025, a child was left inside the childcare program unsupervised for approximately a minute or two while their assigned teachers and classmates were outside for snack. The child walked away from their group at the door leading outside of the Voyager room, walked through the preschool room and into the motor room where they were found playing alone. The program reported this incident to the Child Development Division. A violation is being cited due to the child's age, the length of time they were missing and the breakdown of systems that let a child wander away. Corrective Action: Continue with Action Plan that the program designed that involves using wagons to transport younger children in and out of the program. Children are to be counted at all transitions and staff must use walkie-talkies to radio how many children they have as they exit and enter new classrooms. Teachers are assigned children within the group and directors complete spot checks to ensure compliance.”

    • Background checks

Regulation 6.2.6.6: The program director shall ensure that no person be left alone with children without approval from the Division based on the results of a background check that includes fingerprinting; with the exception of a parent may be left alone with his/her own child(ren).

“The program, upon request to review the staff schedule provided Licensing staff with two (2) systems of accounting; a staff schedule and the Bright Wheel system which staff sign in and out of classrooms documenting their arrival and departures with each group of children. This along with staff statements found that staff who had not been cleared to be alone with children were alone with children on 10/9/2024 and 10/14/2024, which is a violation of this rule. During the debrief with Sara LeBlanc and Tatianna LeBlanc they presented another document that had daily changes of the staff schedule called the Daily Schedule. This document did not show which classrooms or time the house support or support staff worked in the classroom, which ultimately did not confirm documented times cleared staff were paired with non-cleared staff. Corrective Action: Effective immediately do not leave staff alone with children or combined with other staff who have not been cleared to be alone with children until cleared to do so through receipt of the official document from the Child Development Division. By 12/20/2024, submit a program improvement plan that identifies how staffing will align with this rule and how that will be communicated to staff. Include in the program improvement plan how the program will accommodate shifts in schedules such as call outs, or other staffing shortages to ensure staff who have not been cleared remain paired with staff who have been cleared. Additionally, include in the plan how the program will document staff schedules which shows their scheduled plan as well as actual times in each classroom to include arrival, breaks and departures.”

    • Health, medication and illness

Regulation 5.6.5: All prescription medication administered by staff shall be in its original container, properly labeled for the child, and administered prior to the expiration date on the container.

“A child's epi pen was not in the original box properly labeled with the child's name or had the label with the epi pen. The epi pen had also expired in April 2024 and staff were unaware that it had expired. Corrective Action: Immediately request the box for the epi pen with the label which has the child's name and expiration date. Maintain future compliance by training staff to only accept medications that are in the original box labeled with the child's name and create a system to review expiration dates on medications and request new medications when they have expired. Submit a confirmation of completion to Licensor [name] by 6/21/2024.”

“The written permission medication form for a child's epi pen was not in the child's file or with the epi pen during this licensing visit. Corrective Action: Immediately have the parent complete the written permission form for the epi pen. Submit a confirmation of completion to Licensor [name] by 6/21/2024.”
