“The facility self-reported the following supervision lapse: While staff person #1 was in the bathroom with a group of children, staff person #2 was responsible for supervising the children waiting in the hallway. Child #1 had just recently been dropped off and was upset. Child #1 was able to walk away from the group unseen and was able to get from the downstairs hallway bathroom, up the stairs, and down the hallways where he was found by staff working in the front office. Staff person #2 noticed the child was missing quickly, found a walkie-talkie, and notified leadership that a child had eloped from the group. At the same time the alert was being made, the child was being returned to the group. It is estimated that the child was unsupervised for approximately 1 minute, and away from the group for approximately 2 minutes.”
