AAC R9-5-309.C. C. A licensee shall maintain on facility premises: 1. A current fire inspection report including documentation of any repairs or corrections required by the fire inspection report; and 2. If there are gas pipes that run from a gas meter to an appliance or location on the facility premises, a current gas inspection report including documentation of any repairs or corrections required by the gas inspection report.

“Findings: Based on the Compliance Officer’s observations and the review of facility documentation, it was determined that the fire inspection report on file expired 8/13/26.”

AAC R9-5-512.B. B. A licensee shall maintain facility premises and furnishings: 1. In a clean condition, and 2. Free from odor.

“Findings: Based on the Compliance Officer’s observations, it was determined that were wooden art racks stored in the restroom of Classroom A and Classroom B.”

AAC R9-5-518.D. D. A licensee shall ensure that each enrolled child participating in a field trip is wearing in plain view a written identification stating the facility's name, address, and telephone number.

“Senior Compliance Officer reviewed the field trip form dated 9/11/24 to St. Andrew's Church. SCO asked what type of identification the children wear stating the facility's name, address and phone number. According to the Director the children do not wear any form of identification.”

AAC R9-5-518.E. E. A licensee shall also ensure that each enrolled child is wearing out of view a written identification stating the enrolled child's name.

“SCO reviewed the field trip form to St. Andrew's Church dated 9/11/24. SCO asked the Director if the children wear written identification stating the enrolled child's name. The Director indicated that the children do not wear identification stating their names.”
