“When staff are supervising children, they shall always ensure their care, protection, and guidance. Based on review of video footage on 11/20/2025, Teacher # 1 was observed picking up Child #1 (4 months old), who was buckled in an infant bouncer chair. Teacher #1 picked up the bouncer chair with Child# 1 still buckled in and began walking across the room. Teacher #1 tripped over a toy and fell, dropping the bouncer with Child #1, to the ground. The bouncer chair landed hard onto the ground, back into a stationary position. Child #1's head can be seen in video footage bouncing up and down off the bouncy seat, and his entire body lifting up from the bouncy seat when the chair landed. Warning label on the back of the infant bouncer chair read "NEVER lift or carry baby in bouncer".”
