“Based on observation and interview, the center failed to ensure all areas of the premises accessible to children are free of obvious injury hazards to include maintaining cushioning material under playground equipment. Evidence: 1. There was a container of Clorox disinfectant wipes on a table accessible to children. Staff confirmed the wipes were kept on the table. 2. On the playground there was no cushioning material under the fireman's pole which was more than 5ft tall. Staff confirmed there was no cushioning material under the pole area.”
