“LS observed an aide in the outdoor activity area with a group of three year old children. There was no qualified teacher present. The operator shall assign qualified child care teachers to each group of children as needed to meet the requirements for group size and staffing set forth at Regulation .03 of this chapter.”

The operator shall within 5 working days of it's occurrence, provide written notification to the office about the addition of a new employee that includes the individual's full name, date of birth, and date of hire, information about the individual's work assignment, proof of compliance with the laws and regulations pertaining to criminal background checks, and signed and notarized permission to examine records of abuse and neglect of children and adults for information about the individual. This was corrected on site when LS received the staff documents.”
