Evidence - 1. On the date of inspection (08/30/2023), an immunization page for Child #5 was not available for review. 2. The start date for Child #5 was listed as 07/19/2023. Child #5 was in attendance on the date of inspection. 3. The Provider stated (during an interview conducted on the date of inspection) that she thought she had received it from the child's parents and it was included within the Health Forms on file. Documentation of a physical examination and other parts of the Health forms packet were shown to be complete. However, the immunization page on file within this packet was left blank.”
