“Areas inside and outside the center were not maintained in a safe condition. In the toddler classroom, only one of the two toilets was functioning, and the left arm of a brown couch had an exposed screw. The toddler’s playground had three wooden benches with peeling paint, and on the children’s house playground, the three wooden picnic tables also had peeling paint. There were several areas of peeling paint on the walls in Classroom 6 and Classroom 3. On the Children’s House playground, seven plastic borders surrounding the playground equipment were cracked, with two nails protruding through three of the borders. On the Toddler, Nido, and Children’s House playgrounds, the fall zones for the slides and climbing stairs did not contain resilient surfacing. The doorknob on the outside of Classroom #3 leading to the playground was missing, three cabinets in the classroom needed to be screwed back into place, and the sink cabinet door was missing.”
