AAC R9-3-309.B.3. B. If prescription or nonprescription medications are administered at a child care group home, a certificate holder shall ensure that: 3. The designated staff member does not administer a medication to an enrolled child unless the child care group home receives written authorization on a completed Department-provided authorization form that includes: a. The child’s first and last name; b. The name of the medication; c. The prescription number, if any; d. Instructions for administration specifying: i. The dosage, ii. The route of administration, iii. The first and last dates that the medication is to be administered, and iv. The times and frequency of administration; e. The reason for the medication; f. The signature of the child’s parent; and g. The date of signature; and

“Findings: Based on a review of two medications, facility documentation and the Compliance Officer's observation, there were no medication permission forms available for review for Medication #1 and #2.”

AAC R9-3-401.B.2. B. A certificate holder shall ensure that: 2. The areas of the child care group home approved for providing child care services are maintained free from hazards;

“Findings: Based on Compliance Officer #1's and the Bureau Chief's observations, the following hazards were found on the outdoor playground: 1) The green slide was lacking one of the large bolts and was not fully attached to the wood platform structure at the top; 2) There was a large metal pole 3 feet high in the yard by the yellow slide.”
