“Disposable diapers shall be disposed in a leakproof plastic-lined storage system that is foot-operated; used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. In Classroom #1, the foot operated trashcan used for disposable diapers was closed with a childproof locking mechanism. The foot pedal was operable, but Staff #4 used their hands during disposal.”

“Based on interviews, written statements and video, the Center failed to ensure staff who are supervising children, always ensure their care, protection, and guidance. EVIDENCE: On Tuesday, April 30, 2024 at approximately 12:15 pm, Staff #1 was escorting the 3 year old classroom outside to the playground. According to the video, Staff #1 and the children stopped at the corner of the gymnasium building when Staff #1 grabbed Child #1 by the wrist, picked them up off of their feet and carried them approximately 10-15 feet around the corner of the building because of their misbehavior.”

EVIDENCE: On Tuesday, April 30, 2024 at approximately 12:15 pm, Staff #1 roughly handled Child #1. Video revealed that Child #1 was grabbed by the wrist, lifted off of their feet and carried approximately 10-15 feet around the corner of the building because of their misbehavior.”
