“LS noticed that medication had been administered to a child without the proper paperwork being completed by the parents giving permission for the center to give the child in care the medication. LS discussed with the director although she had the completed physicians order she must obtain written consent from the parent on the OCC approved from. The director must show evidence that the approved OCC form has been completed along with the doctors orders.”

“LS observed 2 aides [name] and [name] that did not complete the original Basic Health and Safety Training within the allotted 90n days of employment. However the director did state that one of the Aides in questions has submitted her 2 weeks notice and it is not likely that [name] will complete the required training. LS informed the director to let her know when her last day becomes official and this will be noted at the time.”
