“Based on observation and an interview with a Center Representative (CR #1), the center failed to maintain a sandbox on the playground in a safe and operable condition. Evidence: The sand box was approximately 4 feet by 4 feet. It was buried in the ground with the sides protruding above ground level just a few inches. It contained no sand which left a large, square hole approximately 8-10 inches deep that presented a tripping hazard. CR #1 reported that they no longer have the lid and that they were planning to remove it and fill in the hole with mulch.”
