414.15(c)(12) The program must maintain on file at the program, available for inspection by the Office or its designees at any time, the following records in a current and accurate manner: a current daily schedule documenting the arrival and departure times of each staff person and volunteers;

414.15(b)(22)(iii) The school age child care program must post or display conspicuously in a place to which parents have free and daily access, the following: the address and telephone number of the appropriate regional office of the Office which may be contacted to lodge a complaint against the school age child care program for violations of statutory and regulatory requirements; and

413.4(a) The following criminal history review and background clearances shall be conducted pursuant to and consistent with the Child Care and Development Block Grant Act for any prospective operator, director, employee or volunteer at a child day care center, small day care center, school-age child care program, group family day care home or family day care home, as defined in section (2) of this part, and any person age eighteen (18) or older living in a prospective, or who begins to live, in a group family day care home or family day care home:

414.15(b)(22)(iii) The school age child care program must post or display conspicuously in a place to which parents have free and daily access, the following: the address and telephone number of the appropriate regional office of the Office which may be contacted to lodge a complaint against the school age child care program for violations of statutory and regulatory requirements; and

414.15(b)(22)(ii) The school age child care program must post or display conspicuously in a place to which parents have free and daily access, the following: the name(s), addresses and telephone numbers of person(s) with the legal responsibility and administrative authority for the operation of the program;

414.5(m)(2) 911 and the Poison control phone number must be posted conspicuously on or next to all stationary telephones designated for the school age child care program's use.
