“One infant did not have a written individualized activity plan. The center will need to prepare a plan in consultation with the parent and once completed send a copy to the Licensing Specialist.”

“A complaint was received that the center is not implementing a documented feeding plan. There is documentation in the child's file that she has trouble swallowing, requires "soft bite sized foods", has a specific supplement at breakfast and lunch, and must be fed by staff. While staff verified the child ate her parent-provided food, they did not feed her, operating under the misunderstanding that they only needed to be nearby. The parent was interviewed at the center and clarified that the child can self-feed but requires direct supervision. The LS reviewed the video and observed the child eating a snack alone at a table with her back to the teacher. There were staff in the classroom supervising the children but because staff at that time did not direct feed or provide direct supervision, the center did not follow the feeding plan. The parent agreed to obtain an updated plan explicitly stating the child can self-feed but requires 1:1 direct supervision.”

“A complaint was received that a child was not fed according to the documented feeding plan. The parent observed this on a video that the center posted on the center app. Consequently, the parent, out of precaution took the child to urgent care to be sure the child's lungs were clear as she is at risk for aspiration. The center did not notify this office of this occurrence. The office is aware now.”

“All infants and toddler's files and a sample of other children files were reviewed and within this sample there were several that are missing updated immunizations. Please obtain documentation of updated immunizations and send a copy to the Licensing Specialist.”

“All infants and toddler's files and a sample of other children files were reviewed and within this sample there were several Health Inventory I forms that the parent did not answer the questions listed. Please have the parent complete the missing information and send a copy to the Licensing Specialist.”
