“Based on review, 0 of 20 staff files indicated that there is not a person on site who has been trained to perform a daily simple health screening and the exclusion of sick children. Evidence - 1. On the date of inspection (05/17/2024), it was determined that there was not a staff person who had completed the Daily Health Observation (DHO) training. 2. This finding was confirmed by the Center Director on the date of inspection.”

“Based on review, 3 of 20 staff files indicated that the center did not obtain a copy of the results of a search of the child abuse and neglect registry ( CPS ) from any state in which the individual has resided in the preceding five years before reporting to their start date. Evidence - On the date of inspection (05/17/2024), the following staff did not have an out-of-state CPS check on file at the RE-CDC prior to beginning work: 1. Staff #4 (start date - 08/25/2022) 2. Staff #7 (start date - 09/01/2022) 3. Staff #16 (start date - 10/21/21)”

“Based on review, 1 of 9 children files indicated that the facility did not ensure that all children in the center were in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases. Evidence - 1. On the date of inspection (05/19/2023), an immunization page was not available for Child #7. 2. The start date for Child #7 was listed as 09/06/2022.”

“Based on review, 1 of 13 staff files indicated that not all fingerprint check (FPC) have been completed within the past five (5) years. Evidence - 1. On the date of inspection (05/19/2023), a FPC for Staff #11 was dated - 03/14/2018. 2. The director confirmed that this was the most current FPC on file for this staff person.”

“Based on review 1 of 16 staff files indicated that the facility has expired background documentation that is older than 5 years. Evidence - 1. On the date of inspection (05/17/2022), the Search of the Central Registry ( CPS ) check for Staff #9 was dated - 11/08/2016. 2. The Sworn Statement or Affirmation form (SDS) for Staff #9 was dated - 04/28/2017.”
