“At the time of the inspection, there were items in the pack-in-play while the infant was sleeping. These items included toiletries that were contained in a soft pouch and a sturdy thin mat that was placed under the child. Upon discussion of these items being in the sleep source, the provider removed all of the items that were in the pack-in-play. The provider was reminded that no items of any kind can be in a pack-in-play. She stated she understood.”

“There were no items displayed in the living room. The provider placed items in the living room during the inspection. The provider was informed that this area needs to be set up with materials and activities, especially since this is a main child care area. Suggestions were provided at the time of the inspection. The provider stated she had put away the materials and activities the night before in the other approved child care room. This room however is not currently approved at this time because it is not accessible. A follow-up inspection will take place to reevaluate this room.”

“At the time of the inspection, a child (age 4) was in an unapproved bedroom sitting on the floor with minimal light. It was believed this child was placed in this room because the provider requested that this inspection take place another day since they were in the process of renovating their kitchen. Regarding this situation, even though the provider may have known where the child was, there was still risk for this child being allowed to be in an unapproved room. The child could have wandered in the room and found items that were potentially hazardous, toxic or dangerous. The child was out of sight at the time that the Licensing Specialist was talking to the provider and approved substitute at the beginning of the inspection. At no time should a child occupy space that is not approved by the Office of Child Care and the Fire Marshal's Office. Once the child was retrieved from the room, the child was brought into the living room. The Licensing Specialist requested that the provider gather items for the child to play with since these items were not present in the living room during the inspection. Once this occurred, the child began to play an appeared to be content.”

“There needs to be a small barrier in place outside to prevent accessibility to the side of the yard that contains the burning barrel. The provider stated that the original barrier was moved to another area of the background.”
13A.15.05.04A(1)
“At the time of the visit, there was a four-year-old child sitting on the floor in an unapproved bedroom. There was minimal natural light in the room since the blinds were closed. Child care children are not permitted to utilize areas of the home nor bedrooms that are not approved,”

“The lead screening was missing from the children's files for both children enrolled. A blank lead screening form was given to the provider at the time of the inspection. Please give the form to the parent so that it may be completed by the child's pediatrician. Once completed, return a copy of the form to the Licensing Specialist and keep a copy on file.”
