“The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. On 9/03/2026 at approximately 10:15 am, Child #1 was not signed in on the home's attendance log. The provider acknowledged that Child #1 had been in care for over two hours.”

“The provider did not meet the criteria on the health and safety checklist at the time of the monitoring visit. 1. The provider did not practice emergency evacuation drills monthly to the point of exit from the home. A drill was not conducted in June and July of 2026. 2. The refrigerator must be kept at no more than 40 degrees. The refrigerator which stores children's food from home did not contain a thermometer needed to ensure food is stored at the required safe temperature. 3. The record of Child #1 was missing the required signed acknowledgment of the written notification of whether the home based child care is covered by liability insurance, along with amount if applicable. The provider acknowledged that the insurance information and a copy of the Information to Parents statement and contact information for the inspector or contract agency was not provided to the parent of Child #1.”

“The provider shall maintain an individual record for each child enrolled in care that includes the required information. The record of Child #1, enrolled for over 2 months, did not contain the name, address and telephone number of each parent's place of employment and his or her work hours, the name, address and telephone number of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care, the parent's signed authorization to use a substitute provider and his or her name, address, and phone number, and the disclosure of time a substitute provider will be used.”
