“Repeat violation. Based on interviews with staff the center failed to ensure emergency medications were administered by an appropriately certified staff member as required. Evidence: On 7/28/23 staff #1 administered child #1 insulin as the child's blood sugar was high (380-400). The mother was alerted via the app monitoring the device on the child and she contacted the center at around 11:45am to tell them to administer the insulin immediately. Staff #2 had not arrived at the center to administer the insulin at that time, so staff #1 administered it. Staff #1 has MAT training but does not hold the extra certification to administer insulin. Staff #2 holds the extra certification for administration of insulin, but was not present to do so. It is required that a staff member who holds appropriate MAT certification, based on the type of medications present, be on site and administer the medications to the children.”

“Based on observation and discussions with staff the center failed to ensure that the required infant ratio was met. Evidence: On 7/25/23 at 12:15pm the inspector observed that there were 10 infants, aged 2.5 months thru 16 months, with 2 staff. The required ratio is 1 staff to 4 children. Staff interviews confirmed that the ratio was not met as the 3rd staff member left for lunch break and the replacement staff member had not yet arrived in the room. The 3rd staff member arrived approximately 10 minutes after initial observation of the room. Staff interviews confirmed that this has occurred numerous times a week for the last month.”
8VAC20-780-410-7 Complaint related: Yes
“Based on staff interviews the center failed to ensure that children are not punished for toileting accidents. Evidence: Staff interviews confirmed that while staff #1 and #2 worked together in the classroom they had been witnessed to place children in time-out and yell at them, on numerous occasions, for toileting accidents within the pre-k classroom.”

“Based on an administrative center self report the center failed to ensure emergency medications were administered by an appropriately certified staff member as required. Evidence: Center administration self-reported that staff #1 has been administering an emergency medication (insulin) to a child since June 2023 when public schools released for the summer, through 7/14/23. Staff #1 has the required MAT training, but does not have the add on portion for emergency medication administration as required. Staff #1 and center administration stated that the parent of the child did give permission for administration by this staff and also came in and showed the staff how to administer the medication, but that the staff member has only the regular MAT certification at this time.”
8VAC20-780-350-B-4
“Based on a center administrative self report the center failed to ensure ratios were met for the grouping of children 3 years-5 years as required. Evidence: The center administration self reported that on the morning of 7/13/23, from approximately 10am-10:30am, the ratio was not met for the 3-5 year group on the playground, while a brief lead teacher meeting occurred. Center stated there were a total of 36 children, aged 3-5 years, with 2 staff at the time. Center also stated that there were six 2 year olds also on the playground, with 1 staff. The required ratio for the grouping of 3-5 year olds is 1 staff to 10 children. Therefore, non-compliance occurred for this grouping of children as reported by the center administration.”

8VAC20-780-270-A
“Based on inspection of the building and rooms the center failed to ensure that the areas inside were safe for the children while in care. Evidence: In the 2's classroom there was a mop bucket with dirty water in the bathroom used by the children. The bucket was filled with used standing water; approximately 1/4 of the bucket contained water. This was accessible to the children when they used the restroom. Also, in this same classroom, the closet near the diapering area was unlocked and had several commercial paint cans stored on the floor.”
