“LS observed more than one child that do not have a lead screening at 12 months and 24 months. Center shall review all children files and provide letter of corrections.”

“The complaint states that a staff member leaves children unattended while having their diaper changed on the changing table. According to the OCC approved diapering procedure, a staff member must always keep a hand on the child. A staff member confirmed that she witnessed the staff member in question leave a child unattended on the diapering table to go sit on a cubby of toys. The staff member did not keep a hand on the child. Facility shall review proper diapering procedures with the staff member in question and submit evidence.”

“The complaint states that a staff member videoed infants screaming and crying hysterically for their parents for 2 to 3 minutes while not attending to their needs. Staff and admin team stated that the staff member was written up for creating the video and air dropping it to her phone. The Owner and Director gave copies of the written disciplinary action to the Specialist. Corrected at inspection.”
