“The center Failed to contact the health department about Hand/Foot and Mouth disease. The center had at least 22 confirmed cases. On site, Hand/Foot and Mouth was noted by staff that it started 07/08/2025. The first room with Hand/Foot and Mouth was the two year old classroom in which everyone in the classroom as had it. The center director contacted the health department on 07/17/2025 during the investigation.”

Complaint states Center self-reported that there was an incident during rest period where two 5 year old children climbed under a blanket and pulled their pants down. The two children touched the other's privates. During investigation LS was informed by, the staff that was off the clock that it had to be at least fifteen before the children were discovered under the blanket.”

Other staff members reported to LS that they have witnessed this teacher pinning other children to the wall holding the children's hands on the wall, but not putting her knee in their backs. Staff must ensure that children are safe at all times.”
